The player's unit from the Earth Defense Force, led by "Serious" Sam Stone, is sent to Carcassonne to find the Sirian Artefact of Might (S.A.M.), a powerful ancient relic that may aid against the invasion. The army of Lord Achriman, one of Mental's generals, overruns Rome, forcing Sam and his companions to retreat to Pompeii. There, they remotely trigger an eruption of Mount Vesuvius using the HAARP complex to disrupt Achriman's air forces. Later, in Carcassonne, France, Sam confronts and kills Lord Achriman to avenge his friend Kenny. General Brand, revealed as a traitor, is killed by Sam before he can offer Aaron's Rod to Mental; Sam takes the rod and uses it to defeat Ugh-Zan VI, one of Mental's most powerful warriors, and destroys the superportal by launching an ICBM into it.

There is no cover here, only the open sky and the relentless push of Mental's army. Sam raises his shotgun, knowing that hesitation means death. He remembers Kenny, killed by Achriman in Carcassonne, and Jones, killed by an alien monster during the Pompeii mission. He fights on, eventually killing Ugh-Zan VI with the golden sceptre and strapping the traitor Brand to an ICBM launched into the portal, destroying it.
